Project CHAMPS Housing Project Coordinator & HOPWA Case Manager Lubbock Full-Time

SPCAA Project CHAMPS is a medical treatment and non-medical social service program for individuals who are HIV positive.  Funding is provided by Texas Department of State Health Services (TDSHS) through StarCare Specialty Health System.  This is a full-time salaried position reporting to the Project CHAMPS Program Director.  Regular work hours are 8:00 am to 5:00 pm; however extended hours may be required.  Some travel will be required.  Must be able to adapt to flexible work schedules as established by management.  This position provides programmatic oversight for the HOPWA Program in Lubbock and El Paso as well as HOPWA housing case management assistance and non-medical case management to eligible Project CHAMPS Lubbock clients.  The Housing Project Coordinator serves as the primary liaison for the HOPWA Program in both Lubbock and El Paso between Project CHAMPS and the Administrative Agency in efforts to ensure quality and continuity of housing services to eligible clients in both HSDAs (HIV Service Delivery Areas).  This position also provides HOPWA (Housing Opportunites for People With AIDS) housing assistance and non-medical case management to eligible Project CHAMPS Lubbock clients. This position works with eligible clients to develop Comprehensive Housing Plans and follows up with clients to ensure compliance with programmatic rules and regulations. This position also assists clients in applying for Section 8, Public Housing, and other affordable housing options available in the community.  Works in conjunction with Project CHAMPS Medical Case Managers, Non-Medical Case Managers and contracted physicians to provide daily coordination of all  services and assists clients in accessing other needed health and social services, primary healthcare, and specialized medical services for Project CHAMPS Lubbock clients.  Minimum educational requirements are a Bachelors Degree in Social Work, Sociology, Public Health, Public Administration, Human Services, or other related field.  Knowledge and experience working with people with HIV/AIDS is required.  Knowledge and experience working with the HOPWA Housing Program, Section 8, Public Housing, HUD Programs or other Housing Programs is preferred.  Minimum of 3 years Case Management experience is required.  Minimum of 2 years of Supervisory experience is preferred.  Excellent interpersonal, communication, and organizational skills required.  Ability to communicate effectively with all levels of employees and management, both orally and in writing using correct business English, including spelling, grammar, and punctuation.  Ability to effectively present information and respond to questions in English.  Ability to also effectively present information and respond to questions in Spanish is preferred.
